What it actually does

Double-free of blk_mq_tag_set pointer in device-mapper request-queue initialization path when table load fails, leading to use-after-free and kernel memory corruption on subsequent device removal.

Why it matters to the business

Local users or automated tools can trigger kernel oops and panic on systems using request-based device-mapper targets, causing denial of service and potential data loss or service disruption on Linux hosts.

How an attack unfolds

An attacker with local access issues a device-mapper ioctl to load a request-based table that intentionally or accidentally fails blk_mq_tag_set allocation; the error path frees the structure but leaves a dangling pointer. A later device removal ioctl then calls cleanup which dereferences and frees the pointer again, resulting in kernel panic.

How to fix it

Apply the upstream fix that sets the blk_mq_tag_set pointer to NULL after error-path free in dm_mq_init_request_queue and also in dm_mq_cleanup_mapped_device; update to a Linux kernel containing commit resolving CVE-2021-46938.

How to confirm the fix worked

Confirm the pointer is cleared to NULL on allocation failure paths via source review or kernel debug; test table-load failure followed by device removal no longer produces double-free oops.

Typical attack path

  1. Obtain local shell or process with privileges to issue DM ioctls
  2. Craft or trigger a dm table load for a request-based mapped device that causes blk_mq_tag_set init to fail
  3. Issue device removal ioctl on the partially-created device
  4. Kernel executes double-free path in dm_mq_cleanup_mapped_device leading to oops/panic

How to detect it

Search kernel logs and dmesg for the exact strings emitted during the failing table load and the subsequent double-free oops; inspect call traces containing dm_mq_cleanup_mapped_device, blk_mq_free_tag_set and kfree.

  • device-mapper: core: Cannot initialize queue for request-based dm-mq mapped device
  • device-mapper: ioctl: unable to set up device queue for new table
  • dm_mq_cleanup_mapped_device
  • blk_mq_free_tag_set
  • Unable to handle kernel pointer dereference in virtual kernel address space

What your security team should do

  • Prioritize kernel updates on all systems exposing device-mapper request-based targets to untrusted local users
  • Restrict unprivileged access to DM ioctls via LSM or device permissions where feasible
  • Monitor for kernel oops containing dm_mq_cleanup_mapped_device and blk_mq_free_tag_set in production workloads

The record

In the Linux kernel, the following vulnerability has been resolved: dm rq: fix double free of blk_mq_tag_set in dev remove after table load fails When loading a device-mapper table for a request-based mapped device, and the allocation/initialization of the blk_mq_tag_set for the device fails, a following device remove will cause a double free. Fix this by setting the pointer to NULL in dm_mq_init_request_queue() error-handling. Also set it to NULL in dm_mq_cleanup_mapped_device().
