What it actually does

Improper validation of Smart Install protocol messages on TCP port 4786 allows an unauthenticated remote attacker to trigger a buffer overflow in Cisco IOS and IOS XE, resulting in device reload, arbitrary code execution, or an indefinite loop leading to watchdog crash.

Why it matters to the business

Successful exploitation can cause prolonged network outages, loss of control over routing and switching infrastructure, and potential full device compromise; the vulnerability remains actively exploited years after disclosure with a past CISA KEV deadline, amplifying operational and incident response costs across organizations using affected Cisco devices.

How an attack unfolds

Attackers identify exposed Cisco IOS devices listening on TCP 4786, craft and send malicious Smart Install messages that bypass input validation to overflow a buffer, and either trigger denial of service via reload or achieve code execution; documented campaigns including exploitation of Cisco CVE-2023-20198 alongside this CVE demonstrate continued in-the-wild use by threat actors targeting network infrastructure.

How to fix it

Disable the Smart Install feature entirely on all Cisco IOS and IOS XE devices using the 'no vstack' configuration command; upgrade to fixed software releases that address CSCvg76186 when available; restrict access to TCP 4786 via access control lists; monitor vendor security advisories for per-version patches.

How to confirm the fix worked

Verify that the 'vstack' feature is disabled in running configuration; perform network scans for unexpected listeners on TCP 4786; test device stability under simulated Smart Install traffic in a lab environment; review Cisco Bug ID CSCvg76186 release notes for the specific fixed IOS versions applicable to each platform.

Typical attack path

  1. Scan for Cisco IOS/IOS XE devices with TCP 4786 open and Smart Install enabled
  2. Send crafted Smart Install protocol message containing malicious payload
  3. Trigger buffer overflow leading to control-flow hijack or memory corruption
  4. Execute arbitrary code for persistence or force device reload for DoS

How to detect it

Search device logs and syslog for Smart Install protocol events, unexpected reloads, or watchdog crashes on TCP port 4786; inspect network flow data for inbound connections to port 4786 from untrusted sources; review historical incident reports tied to the listed threat actor campaigns exploiting this and related Cisco vulnerabilities.

  • TCP connections to port 4786 containing Smart Install protocol headers
  • Syslog messages referencing vstack or Smart Install message processing errors
  • Unexpected device reloads or watchdog timeout entries in crash logs

What your security team should do

  • Immediately disable Smart Install on all internet-facing and internal Cisco devices
  • Prioritize patching or feature disablement for devices in the CISA KEV catalog
  • Implement network segmentation and ACLs to block unsolicited Smart Install traffic
  • Deploy monitoring for anomalous traffic on TCP 4786 and device crash logs
  • Conduct inventory of all Cisco IOS/IOS XE assets and their current software versions

The record

A vulnerability in the Smart Install feature of Cisco IOS Software and Cisco IOS XE Software could allow an unauthenticated, remote attacker to trigger a reload of an affected device, resulting in a denial of service (DoS) condition, or to execute arbitrary code on an affected device. The vulnerability is due to improper validation of packet data. An attacker could exploit this vulnerability by sending a crafted Smart Install message to an affected device on TCP port 4786. A successful exploit could allow the attacker to cause a buffer overflow on the affected device, which could have the following impacts: Triggering a reload of the device, Allowing the attacker to execute arbitrary code on the device, Causing an indefinite loop on the affected device that triggers a watchdog crash.
